suggest moving PipelineStream to it's own module

Even if you're using it as part of vstream it's purpose is quite different,
if I want an abstraction for packaging a pipe line that's a module I'd go
looking for, but if I want to be able to instrument streams for debugging
then that's a separate thing again.
